In case I have to inform your GP of the advice I have given you, I will ask for your consent. This is to confirm that you are happy for me to do so. If we hold a conversation or consultation via Skype, FaceTime or telephone, I will ask you to send me your signed consent form via email and to confirm in your email that you give your consent.
